to Unlicensed Personnel (Home Health Aide) services are provided by and under the supervision of a Registered Nurse and include assistance with medications, treatments and therapies.
o Assistance with Activities of Daily Living, including the following.
i. Bathing
ii. Dressing
iii. Grooming
iv. Oral Hygiene
v. Hair Care
vi. Eating/Self-Feeding
vii. Transferring
viii. Continence Care
ix. Toileting
o Mobility support
o Providing standby assistance with arm’s reach for safety while performing daily activities
o Cueing/reminders for self-care
o Providing verbal or visual reminders to take regularly scheduled medications, treatments and exercises
o Eating assistance for those residents with complicated eating problems
Homemaking and Companionship services include the following.
o Housekeeping
o Meal Preparation (3 nutritious meals/day plus snacks), including modified diets ordered by a licensed health professional.
o Laundry
H&S Residence staffs an awake caregiver who is responsible for responding to the requests of residents for assistance with health or safety always needs, including overnight. Residents are provided with a system for accessing staff when needed.
o Daily Checks for all residents
o Security and monitoring
o Assistance with arranging transportation for personal, social and recreational activities.
o Assistance with arranging transportation to medical and social services appointments.
o Assistance with accessing community resources and social services.
o Daily socialization and recreation
o Culturally sensitive programs and activities
Assisted living services are available 24 hours/day 7 days/week. Normal office hours are 8:00 AM to 4:00 PM Monday through Friday. After-hours assistance is available by calling 763-306-2336.
o When a referral is received for a resident with complex medical and/or behavioral needs, the RN will determine the Facility’s ability to provide the level of service required. If the resident is admitted, the RN is responsible for the following activities.
o Establishing protocols for the care(s) required
o Assuring that all staff providing service to the resident are competent to provide the task(s)
o Orienting all staff providing service to the resident to the individual care plan
o Supervising paraprofessional staff according to H&S Residence policy and/or nursing judgment
o Evaluating the appropriateness of the protocol for the resident and updating it as indicated
